Who Takes Care of Me
Promise uses an interdisciplinary team approach with a personalized treatment plan that is based on the severity and complexity of your medical condition. Our aggressive clinical and therapeutic intervention involves daily physician monitoring, 24-hour RN care, services located at the facility so that you do not need to be transferred back and forth for tests or therapy, and the appropriate medication regimens.
Our hospital staff works closely with your referring physicians and case workers to discuss the individual treatment plan you require. To effectively address your needs on all levels, including physical, emotional, social and spiritual, our team can include:
| • Physicians • Registered Nurses • Licensed Nurses • Certified Nurse Assistants • Occupational Therapists • Speech Therapists |
• Respiratory Therapists • Wound Care Specialists • Social Workers • Pharmacists • Dieticians • Pastor, Rabbi, Priest or other spiritual practitioner as requested |
Family and friends are valued as essential team members to support and encourage you, not only during your stay at Promise, but also after discharge from our hospital. From the moment of admission, our staff proactively involves you and your family, helping them understand the specific care that will be provided, and enlisting their active participation in the recovery process. Our philosophy recognizes that education for you and your family is essential to managing your present and future healthcare needs.
Depending on your personalized physician-directed recovery plan, discharge from a Promise hospital is usually to your home, a nursing home, or a rehab skilled nursing facility.
